Speaking of which, they showed a replay of a Mitch no-ball earlier, and the batsman at the non-strikers end (I think Cook) was well out of his crease and just strolling forward as Mitch delivered ... Is it bad etiquette for the bowler to give him a little warning?

Believe they are still allow to Mankad them providing the bowler is not yet into his delivery stride? See it all the time these days, esp in one day cricket, but rarely moving as early as Cook was.
Also, think they allow batsmen to do it because of the video replays where it was felt batsmen were disadvatantaged by being given out by the bare margins, especially in ODI's.

This umpire does struggle i think, billy doctrove is it? he was the one that missed the KP's lbw appeal in the first innings aswell.
I dont think it was very poor, just going over the top, so shoulda been not out, but it wasnt like ala Martyns decisions in the 05 ashes
