We used to get a flat 10 days, but since we changed awards I think it now accrues... but at Telstra we had "unlimited" sick leave

it all depends on the agreement between the employer and employee. All agreements have to be based on the award though, so a employer can't make you to agree to any terms below the award. So I'd find it hard to believe that a employer is able to wipe your sick leave.

Some employers have agreements where they pay or all or some of the remaining sick leave at the end of the year.

Where my brother used to work they would pay you out on any leave above 5 days that hadn't been taken and you would accrue the up to 5 days left, if you took none they would let you accrue 5 but as a bonus, pay you out the value of the full 10 days, biggest problem was in the last month or so everyone who hadn't had a day of yet would come to work sick, most years most of the work force was sick for xmas

When I last worked as a full time employee (of a public hospital in SA) any more than 2 consecutive days off required you to produce a medical certificate.
I had a few hassles with the administration clerk one Friday long weekend when I was rostered off, and was then sick on the Monday I was due to go back to work.
He didn't accept the argument I had performed my rostered duties over the long weekend - namely rostered off duty - and my pay was docked.
I had to go over his head to get it sorted out. Fortunately his boss accepted my reasoning.

IIRC from my reading of the history, the original concept was that sick leave is there for when you were genuinely sick only and a certificate could be required for even one day. I don't think it accrued in those days.

However, over years it has been relaxed and most employers do not require a certificate for one or two days.
I understand some union deals, or under the Howard government negotiated "Enterprise Agreements", did allow for sick leave to accrue over years, and be paid out, but it is not universal. So, there seems to be a lot of variation now.

However my knowledge is mostly gleaned from being the doctor asked to write certificates.
(And sometimes declining to when I had no grounds I could objectively determine to justify it.)
