4 minutes per over is the allowance making it 15 overs per hour.
Sessions are 2 hours long with a 5 minute drinks break at the umpires discretion in consultation with the captains. Last over of the session must commence before the scheduled break time.
3 sessions of 2 hours equals 90 overs this doesn't take into account lost time for wickets taken (2 minute allowance) or waiting for DRS to conclude.
4 minutes per over is only mentioned in the recalculation for lost time. At no point anywhere in the ICC playing conditions for test cricket does it mention 4 minutes per over under normal play.
6 hours for 90 overs, that's the expectation, and it includes all the 'expected delays' (a normal number of wickets, moving sight screens, fetching the ball from the boundary, three drinks breaks, etc).
