The thing that triggered this was hearing Nirvana's "Lithium" on the MP3 the other day (not a hilarious event on its own, normally). Was at a 21st, a few years after this had come out. The guest of honour was 6"4, football type and as expected for that sort of event, had quite a few coldies on the night.
Someone cranks up "Lithium" and everyone is getting into it. The birthday boy the most of all. Is standing in a doorway when the chorus really starts getting loud. Forgetting where he is, he jumps vertically (he was a ruck sort and had a pretty fair spring on him) with full enthusiasm.
I was standing 20 feet away and heard the crack of his head on the lintel as if it was next to me. Note - he was absoutely fine, no ill effects, but the whole party just dissolved into laughter. Everytime I hear that song now, it brings a smile to the face.

Seen a few incidents over the years but the one that still stands out was at a Painters and Dockers gig at the Tivoli in 1990.
As usual at a P&D gig quite a few of the punters were stage diving, but there was this one guy that was just doing it over and over and over. He'd get up on stage - dive - be carried through the audience and then get straight back on stage and do it again. After about 10 times the guys down the front were getting pretty pissed off with this guy so the next time he did it as soon as he jumped the crowd parted like the red sea. He hit the floor with an almight thud (about 3 feet in front of me). I'm thinking "Shit! This guy is going be really badly injured"" next thing he jumps up throws his arms in the air and let's out a massive "WOOOOOOH" with a big smile on his face.
I suspect he may have had more than beer than night!

Mike Patton of Faith No More daring a fan to jump from the balcony at the Old Greek Theater in Melbourne August 1990.
"Sue us, we're rich!"

Was at the Indy Fest back in 1997 (I think) at one of the uni's in Adelaide.
TISM were playing at the time and there was a bloke watching from a tree, with his legs wrapped around the branch.
The security asked him to get down and he refused so they grabbed him by the legs and pulled him down.
Unfortunately they both pulled at the same time which proved very painful for the poor fella who I reckon would have had sore nuts for a week after that.
Also saw a bloke jump off the speakers during the Living End concert at the same gig.
Everyone was telling him to jump and that they'd catch him.
When he did, everyone moved out of the way and he landed flat on the ground before being dragged out by security.
Great times!

I had both my steel toes torn out of my boots in the crazy mosh pit when RATM played at BDO.
Speaking of shoes, I remember another time a fella next to me suddenly losing his shoe but the packed and swirling mosh stopped him from picking it up. Show ends, crowd disperses and I hear someone shout "I FOUND MY SHOE!" and turn to see this same guy with his battered sneaker triumphantly raised in the air.

I saw Chocolate Starfish

at the Crown, Victor Harbour, and was right at the front of the stage. Adam Thompson pointed and me and gestured for me to club on stage. Having had a few I shrugged and climbed up ! A bouncer came from no where and grabbed at me and pulled me off stage. Next thing they've stopped the song, told the bouncer to **** off, and got me back on stage. Thompson grabbed my shirt, ripped it in half, and then gave me a really soft Liverpool kiss whilst singing the song "Head". I've done the right thing and gave him one back (softly of course), turned around ripped off my trashed shirt and jumped off stage.
After the set, Thompson found me and bought me a few drinks and I chewed the fat with the band for a while in a very pissed state...funny !

When i went to Slipknot/Segression gig several years ago now, I was in the mosh pit and some staunch looking mother sucker was up on the balcony.. Stood up there with his Pen1s hanging out, (Not sure what he was doing i.e p1ssing etc) but then he puts it away and jumps knees first into the mosh pit from top balcony.
Very humerous
