I missed the whole explanation, but O'Hara can't come back onto the ground because he didn't come off through the interchange area.

I've just heard via radio that 2/3 players named on the interchange bench actually started on the ground therefore meaning two illegal interchanges therefore down to one interchange player only. Is this true??

If it is true, that is an incredibly basic mistake....
